Registration Form
A copy of our registration form can be found below to download. It should be returned to maneapreschool@manea.cambs.sch.uk. Alternatively a paper copy of the registration form can be collected from the Manea School office.
PLEASE NOTE THAT WE ACCEPT REGISTRATION FORMS ONCE YOUR CHILD REACHES 18 MONTHS OLD
(Also sometimes known as prior injury forms)
If your child arrives at pre-school with an injury e.g. a bruise or graze, then you must fill in an existing injury form. This is a safeguarding requirement and if your child shows us an injury that you have not told us about we will call you to ascertain how the injury happened. You can download the forms here and e-mail them to us, ensuring that whoever drops off your child lets us know one has been e-mailed. Or you can print them off and return at the door. There are forms available outside of pre-school to fill in when you drop off as well. These forms help us to know that it is not a new injury that has happened at pre-school as well.
Medication forms
We hold medication forms in pre-school for you to fill in when your child is on medication.
If you are unable to fill in a form in person on the first day of medication then you can fill in one of the forms below and return to us, either to bring in with your child or to email to us at maneapreschool@manea.cambs.sch.uk
We are unable to administer any kind of medication, for any reason, without one of these forms filled in and signed by someone with parental responsibility.
